Applicant can not have had a bankruptcy in the last 5 years.
All drivers must be licensed.
Drivers that have a major violation (DUI, DWI, Reckless Driving, Excessive Speeding [More than 20 miles over Speed Limit], License Suspended or Revoked, etc.) in the past 3 years are prohibited.
All vehicles must be garaged in the applicant's primary state.
Vehicles leased or rented to others by insured.
Public or private (including nonprofit) livery – limousines, church vans, daycare vans, and taxicabs limousines, church vans, daycare vans, and taxicabs are prohibited.
The transport of hot liquids, hot tar, flammables, chemicals, explosives or hazardous materials is prohibited.
Risk that require ICC, PUC, MCS90 or Hazardous Materials filings are prohibited.
Pizza or other “time sensitive” food delivery is prohibited.
Ambulances, emergency and/or law enforcement vehicles are prohibited.
Amusement devices mounted on vehicles are prohibited.
Concrete ready-mix vehicles are prohibited.
Tank Trailers that haul anything other than water are prohibited.
House (structure or building) movers are prohibited.
Ice cream vendors/dealers are prohibited.
Vehicle leasing or rental firms are prohibited.

Buildings occupied for both dwelling and business purposes, except incidental office, professional, private school or studio occupancy are prohibited. Vacant dwellings are prohibited. Functioning smoke or heat detectors required in each unit. Dwellings occupied as a housing project, a fraternity or a sorority. Swimming pools are to be separately classified and rated. Swimming pools must have 4' fence completely enclosing the pool that has 2 self-closing and positive self-latching closure mechanisms that are at least 45" above the ground, pool rules posted and life-safety equipment available.
